Blake Langermann, an investigative journalist and cameraman working alongside his wife, Lynn, crash-lands and roams onto the Supai region[4] of the Coconino County region of Arizona, while following the mysterious murder of a young, pregnant woman only known as Jane Doe.[2] When he wakes up after the crash, Blake finds the pilot of their helicopter crucified nearby and his wife missing. Blake makes his way to a nearby town, where he discovers Lynn in a chapel, captured by a cult led by "Papa" Sullivan Knoth. Knoth claims that the town, Temple Gate, lies on the mouth of Hell, and that Lynn is pregnant with the Anti-Christ. They escape the chapel, but Lynn claims to suffer from stomach cramps. The pair are eventually separated when Lynn is kidnapped by Val, leader of the Heretics, who wish to hasten the end of days and are in conflict with Knoth's cult.
Blake is rescued by a man named Ethan who has left Knoth's cult. Ethan claims to be the father of Anna Lee, the Jane Doe that Blake and Lynn were investigating. He explains that Knoth rapes the women of Temple Gate and, once they are pregnant, demands that they are executed on suspicion of harboring the Anti-Christ. Fearing this fate for her, Ethan convinced Anna Lee to flee. As Blake rests under Ethan's house, Marta, an imposing woman wielding a large pickaxe and one of Knoth's executioners, breaks into the home and murders Ethan after accusing him of heresy, forcing Blake to flee again. Upon reaching another chapel, while hiding, Blake learns from a tortured heretic being interrogated by Knoth that Val is keeping Lynn in the mines under Temple Gate.
Blake begins suffering from progressively more disturbing hallucinations based on his childhood, all while being pursued by a grotesque monster. The hallucinations gradually reveal a night at Blake and Lynn's Catholic elementary school and one of their childhood friends, Jessica Gray, and the events surrounding her death. Blake and Jessica, while staying late at their school, are caught by one of their teachers, Father Loutermilch, who is implied to have been molesting Jessica. Loutermilch sends Blake away, but as Blake is about to leave the school, he hears Jessica's screams and sees her being chased. Blake discovers her to have fallen down a staircase, breaking her neck, with Loutermilch at the top of the stairs. This reveals that Jessica's presumed suicide by hanging was a cover up by Loutermilch, and the monster that has been chasing Blake throughout the school is a perverse, twisted version of how Blake sees the priest.
Surviving numerous encounters with Marta and Temple Gate's other inhabitants, Blake escapes the town and finds himself in the camp of the Scalled, diseased outcasts from Temple Gate led by Laird Byron, a grotesque dwarf mounting on top of a large brute named Nick Tremblay. Blake is proclaimed the Scalled Messiah, and crucified to be "reborn", although he manages to escape. Laird and Nick are accidentally killed by the other Scalled while chasing Blake. Soon after Blake finds a document revealing that the Murkoff Corporation is the cause for everyone's insanity due to an experimental mind control station hidden deep in the mountains.
Reaching the mines, Blake faces Val and the heretics. He heads deeper into the mines and enters Val's underground temple. At the center of it, Blake finds Lynn, now pregnant. Knoth's cult finally catch up with them and murder the heretics (and presumably Val), allowing the pair to flee. As dawn breaks, a freak lightning storm begins to destroy the town. Marta appears again and attacks the two, but is ironically impaled by a falling metal cross brought down by lightning from the chapel in the distance. Blake and Lynn make their way to the chapel where Lynn gives birth but dies from blood loss soon afterwards. Blake blacks out holding the newborn, which Lynn's last words imply is also a hallucination. Knoth greets Blake as he wakes up. Knoth claims that he had to kill all of his followers, and advises Blake to kill the child. Knoth then slits his own throat. As Blake walks outside, he finds all of Knoth's followers have committed mass suicide in preparation for the apocalypse. The sun grows brighter and Blake is engulfed by the light; he has a final vision of being together with Jessica in school kneeling down and praying.
